Teams


Direct support workers always work alongside other key people. Together, they create and follow a personalised support plan.

 

Who makes up a multidisciplinary team?

 

Support Workers: Provide daily, hands-on help and know the person's routines best.

 

Psychologists: Help with emotional wellbeing, behaviour plans, and mental health.

 

Occupational Therapists (OTs): Help people learn everyday skills and find tools to make daily tasks easier.

 

Physical Therapists (PTs): Help people move around, build strength, and use mobility equipment like wheelchairs.

 

Medical Professionals: Doctors and nurses who look after physical health and medications.

 

Social Workers: Help with rights, housing, funding, and community services.

 

Partners

Partnerships go beyond official medical teams. They include the key people in the individual's everyday life who help make decisions and offer support.

Family Members: Offer lifelong knowledge, love, and continuous support.

 

Friends and Advocates: Stand up for the person's rights and offer personal advice.
